Application forms can be downloaded from our website or obtained in person at our school. The completed form, along with all required supporting documents, should be submitted either by post to the school address (2 Marigold Road, Yau Yat Chuen, Kowloon) or in person. Please ensure sufficient local postage is affixed before mailing.
Required Supporting Documents:
- An application fee of HK$40 can be paid by cash or crossed cheque. For submissions by post, only cheques are accepted. Cheques should be made payable to “Yau Yat Chuen School”.
- Please affix one recent passport-sized photo (1.5″ x 2″ / 3.81cm x 5.08cm, taken within the last 3 months) onto the application form.
- A copy of the child’s Birth Certificate or a copy of the Document of Identity for Visa Purposes / Proof of Residence in Hong Kong (for children born outside Hong Kong).
- A double-sided copy of the Immunization Record (including the cover, which must clearly display the child’s name).
- If the child’s father, mother, elder brother, or elder sister previously attended or is currently studying at our school, please provide a copy of their student handbook (school registry page) or graduation certificate as proof.
- Two 4″ x 9″ stamped, self-addressed envelopes:
- Please affix sufficient local postage to each of the self-addressed envelopes.
- Please write your correspondence address and the child’s name on the envelopes.
Please prepare the following documents for upload:
1. The child’s Birth Certificate (The child must be born on or before December 31, 2024);
2. A digital file of the child’s recent passport-sized photo taken within the last 3 months (The file size must be less than 5MB);
3. The child’s Immunization Record (The child’s name must be clearly displayed);
4. A screenshot of the transaction record for the application fee payment of HK$40.
Payment Methods
You may settle the payment through any of the following methods:
- Faster Payment System (FPS): Transfer to the school’s bank account listed below.
- AlipayHK: Transfer to the school’s bank account listed below.
- Bank Counter: Deposit in person at any branch of The Bank of East Asia (BEA) or Hang Seng Bank.
- Automated Teller Machine (ATM): Transfer via any bank’s ATM.

To ensure accurate verification of your payment, please write the following information clearly on the “blank space on the front of the pay-in slip” or on the “printed transaction screenshot” after payment:
- Applicant’s full Chinese name
- Applicant’s full English name
- Birth Certificate Number
- Payment Date (DD/MM/YYYY)
- Payment Time (HH:MM)

Important Notes
If using electronic transfer (FPS/AlipayHK), please ensure you take a screenshot and print out the transaction confirmation screen. You must retain this pay-in slip or printed screenshot with the filled-in details. Parents must submit the original copy of this document to our school staff on the interview day for verification purposes.
